Waiver of Liability, Assumption of Risk & Filming Consent โ Minor Participant
I am the parent or legal guardian of the minor named in this application ("Participant"). I give permission for the Participant to take part in the 1v1 challenge streaming event described above ("the Event"), organized by Shalom in the Detroit Metro area.
Assumption of risk. I understand that soccer and related physical activity carry inherent risks, including but not limited to physical injury, collision with other players, strains, sprains, fractures, and in rare cases severe or catastrophic injury. I voluntarily assume all such risks on behalf of the Participant.
Release of liability. To the fullest extent permitted by law, I release, waive, and discharge Shalom, the Event organizers, hosts, staff, volunteers, venue owners, sponsors, and affiliated crew (collectively, "the Organizers") from any and all liability, claims, demands, or causes of action arising out of the Participant's participation in the Event โ including claims of ordinary negligence โ whether arising before, during, or after the Event. I agree the Organizers are not responsible for any injury, loss, or damage of any kind connected to the Event.
Filming consent. I consent to the Participant being filmed, photographed, and recorded during the Event, and to that footage being streamed live and/or published online โ including on social media โ without further approval or compensation owed to me or the Participant.
Medical. I authorize the Organizers to arrange emergency medical care for the Participant if needed, with costs being my responsibility.
I confirm I am the Participant's parent or legal guardian, that the information in this application is accurate, and that I have read this waiver in full and sign it voluntarily.
Note: this is a general-purpose template, not legal advice.
